REQUIRED DOCUMENTS FOR CHECK IN
|
The following will be required at check in:
1) proof of age for each player (either a US Youth Soccer player pass or photocopy of birth certificate will do)
2) proof of US Youth Soccer membership for each player (player pass; photocopy of GA Soccer roster; or letter from your league registrar)
3) medical release form for each player (does not need to be notarized and can be your league's medical release form).
4) tournament liability release signed by a parent of each player (located under documents)
5) 4 copies of your tournament team roster, which you build under your access code as a coach or team manager. |
